Use this service to register to use GP online services.
You can use this service if you:
- are registered at the surgery
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- photographic identification and proof of address
Important: Please note
If you submit this request after 4pm on a working day, it will be opened when we re-open on the next working day. The practice is closed over the weekend.
Once you have completed this online form you will need to either upload or visit the practice with two forms of identification. One of these items should include your photograph.
We will then issue you a username and password.
Once you are registered you will be able to use the service to:
- Review your medications and known allergies
- Order your repeat prescriptions
- Make an appointment
- Cancel an appointment
- Change your contact details
