How to request an appointment

You can request an appointment with one of our clinicians using the online forms below. From 7th April 2026, we are moving to a new system whereby all urgent appointment requests will be triaged by a GP. Information:

Our appointment request forms are open Monday to Friday, 7:00am – 5:30pm only. If you have an urgent appointment request from 5:30-6pm, please call reception. We are closed to appointment requests outside of these hours.

Use this form to request an urgent appointment, if you feel you need to see or speak to a clinician as soon as possible. For situations that can’t wait, such as a new illness arising in days or weeks, or a concerning new development in an existing condition.

A GP will review this request promptly to assess the urgency and next steps: if an urgent appointment is needed, this will typically be offered on the same day, or first thing the following day, depending on the time you submit your request.

Urgent appointments are typically 10 minutes long and for addressing a single health issue only.

Availability for urgent appointments is limited by capacity on the day: therefore we encourage you to submit your request as early as possible.

Please ensure you keep your phone on and close by after submitting an urgent appointment request: our reception staff may call, text or email to offer you an appointment slot, or the triaging GP may contact you for more information.

Routine appointments

Routine appointments are for non-urgent medical or nursing issues, such as:

  • Problems which have been ongoing for months or years
  • Management of long-term conditions such as high blood pressure, asthma and diabetes
  • Discussing referrals, paperwork or advice from specialists
  • Blood pressure checks, cervical smears, family planning and more

These appointments are typically offered 1-3 weeks in advance and offer more choice of clinician and timings.

This form is open from Monday to Friday, 7:00am – 5:30pm.

You can also:

  • use your NHS account (through the NHS website or NHS App) or Patient Access to book an appointment, screening test or vaccination
  • phone us on 0207 722 4135
  • speak to one of our receptionists in person

When you get in touch, we’ll ask what you need help with.

Please note that from 7th April 2026, if you phone or attend the practice in person to request an appointment, we will still need to fill in the online appointment request form to capture your request digitally. If you find it difficult to complete the online form yourself, or don’t have internet access, don’t worry: our reception team can still help you over the phone or when you walk in. 

We will use your answers to choose the most suitable doctor, nurse or healthcare professional to help you.
